Dr Carey- I utilized BMP alone in this case but shaped the mesh to act as a scaffold to allow me to advance and support the soft tissue that was advanced by using traditional periosteal release high in the vestibule and passing a slow resorbing suture through the mesh at the papilla junction. Complete correction is not always the case but you can come back and utilize additional grafting techniques such as sub ep. Thank you for your kind words of support HHC

Dr.Martin I really enjoyed your presentation. One of your cases using BMP to augment a defect for #7 also corrected root exposure of teeth #8&6.Did you use BMP alone or with an alloderm blanket. Again enjoyed the presentation and thanks for the sharing of the knowledge. Keep it Going.

Band Thanks- I have used curve with BMP-2 and am waiting on results- about 4 months now- the folks at Zimmer have new recommendations to improve ease of use- I have not used the Medpor contain yet but plan to. I like titanium because of the flexibility - iI have the Artisan system from Medtronic and plan to use very soon. I will post my results. Mo - thanks - I have I 10 month follow up om BMP-2 coming up Miles- I like to use Alloderm if I am not using mesh - I have had small exposures without incident and will just have patient improve home and see more frequently. The key to minimizing exposures is to follow tension free closures as taught by Drs Pikos , Salama, Craig Misch and if using for root exposure review Dr Pat Allen's lectures. .

Nice job Dr. Martin. Can you use BMP-2 with something other than Ti-mesh? Have you used Gortex, Contain from Styker, or Curve from Zimmer as alternatives to Mesh? Thanks Band

Patients often neglect or delay dental care for an extended time because of their negative experiences with and fear of dental treatments. This neglect may result in the advance of periodontitis with severe bone loss, a substantial number of missing teeth, and problems with the remaining teeth. Preexisting malocclusion can aggravate this condition. Fibrin-Guided Bone Regeneration: Guidelines of a Simple, Predictable, & Low Risk Technique
This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.

The Dual Zone Approach to Tooth Replacement in the Esthetic Zone
Improvements in implant designs have helped advance successful immediate anterior implant placement into fresh extraction sockets. Clinical techniques described in this presentation will enable practitioners to achieve predictable esthetic success using a method that limits the amount of buccal contour change of the extraction site ridge and potentially enhances the thickness of the peri-implant soft tissues coronal to the implant-abutment interface. This approach involves atraumatic tooth removal without flap elevation, and placing a bone graft into the residual gap around an immediate fresh-socket anterior implant with a screw-retained provisional restoration acting as a prosthetic socket seal device. Long term follow up and evidence from scientific literature to support this concept will be described.

Advances in Hard and Soft Tissue Management Using 3D Printing Technology
It is becoming rare today to come across simple cases in Periodontics; our specialty has come to face complex cases where hard and soft tissue deficiencies are quite common. These suggested protocols and schematic approaches were developed to help the surgical practitioner visualize and divide the problem into a predictable step-by-step workflow. The utilization of 3-D printed CBCT studies of the treatment planned surgical sites was shown to be integral in patient care and clinical outcomes. Every step is dictated by biology; all incision designs, flap management techniques, and biomaterials used have been selected to maximize the blood supply in the area and minimize trauma to the vascular network that nourishes our surgical site.

En nuestra clinica observamos un aumento de pacientes adultos con problemas esqueletales, compromiso periodontal y necesidades protesicas. Debemos recurrir a la interconsulta con el periodoncista, protesista y cirujano maxilofacial, para la correccion del caso, prestando especial atencion al componente dentario, esqueletal y los tejidos blandos faciales del paciente. La cirugia ortognatica, que generalmente la realizamos luego de una preparacion ortodoncica, permite corregir discrepancias en el adulto y restaurar la funcion y estetica en los tres planos del espacio. Realizada la correccion oclusal, procedemos a sustituir dientes ausentes, mediante implantes y protesis fijas. Fibrinogen-Induced Regeneration Sealing Technique (F.I.R.S.T.). An Improvement and Modification of Traditional GBR: A Report of Two Cases
Guided bone regeneration is a technique widely known, clinicians know very well indications and limitations of this technique. One of the principles to achieve bone augmentation and formation resides in the stability of the blood clot forming under a barrier membrane. The technique proposed in this article has the goal of providing stabilization to the bone graft by adding fibrin sealant (FS) to the bone graft and also using the fibrin sealant to attach a bone membrane (cortical lamina) to the recipient site. This simple modification of the technique of guided bone regeneration is presented in two successful cases.
