Shane; I believe that vital autogenous bone to be somewhat better than any other bone products although we can achieve similar results with alternative bone grafting products such as allograft, xenograft and some synthetics. Surgical skill, proper technique and surrounding bone walls are also key to success. Dr. S

Dr. Salama, Do you feel harvesting particulate grafts intra-orally have a big advantage over particulate allograft materials? What differences are you seeing?
Thanks,
Shane

Wleed; 250-450 RPM with sterile water or saline.

Hi Maurice, what rpm do you use and do you use saline irrigation? Would you consider using it for osteotomy preparation for eg short Implants like BTI?

Maurice Salama - (7/18/2012 6:51 PM)
Mike and Shane; The drill size I use is Large 6.0mm but I use 5.0, 6.0 and 7.0mm as well. Typically they can last around 5 times before replacement. Great tool. You will not be disappointed. You can purchase on this site on the Neobiotech Product page. Good luck Dr. Salama

Pre-Implant Ridge Bioengineering: The Bone Block Allograft Technique - Part 2 of 2
Narrow ridge deficiencies can limit dental implant placement. As well, the pain and various complications associated with chin, ramus and hip bone block grafts for ridge augmentation purposes discourage many from pursuing this method of treatment. The 'bone block allograft technique' helps overcome narrow ridge limitations for dental implant placement as well as minimizing surgical post-op pain, risks and complications. This is an extremely versatile procedure which can be performed in the anterior and posterior sextants of both the maxillary and mandibular arches. Managing Large Odontogenic Cysts; Decompression or Marsupialization Technique Part 2 of 2
Decompression and marsupialization are techniques used to reduce size of large odontogenic jaw cysts, hence making its complete removal and bone regeneration simpler, safer, and more predictable. In this video, Dr. H. Ryan Kazemi demonstrates a simple technique for decompression or marsupialization and provides a brief review of its application in management of odontogenic cysts

Modern Implant Dentistry: Rules of Engagement in the Esthetic Zone - Part 2 of 2
This presentation will focus on the interdisciplinary relationship of the restorative dentist, periodontist and orthodontist to reconstruct the soft tissue foundation for all of these restorative options in anterior tooth replacement. The diagnosis of deficiencies as well as the varied treatment options will be discussed in detail. This includes periodontal crown lengthening, esthetic periodontal plastic soft tissue grafting procedures as well as prescription adjunctive orthodontic tooth movement to manipulate the soft tissue foundation prior to or subsequent with the restorative options of implants, bridges, or pontic replacement.

Advanced Bone Grafting Techniques: Part 2 - Autogenous Bone Blocks in the Reconstruction of the Atrophic Maxillary Ridge
This lecture will describe the use of autologous onlay block bone grafting for reconstruction of moderate to severe atrophic maxillary alveolar ridge. This will include a discussion of the various options to this form of grafting, such as short implants, maxillary sinus augmentation, sub-nasal elevation procedure, connective tissue grafting and soft tissue manipulation, Le-Fort I down fracture/osteotomy and the use of progenitor cells: bone marrow aspirating concentrated.
A description of surgical donor harvest sites in the mandible will also be performed as well as post-op healing of these sites. Additional time will be spent reviewing causes of failure with bone blocks and implants placed into these sites.

Esthetic Zone Reconstruction: Synergy of Hard and Soft Tissue Grafting
The loss of alveolar ridge contour in the esthetic zone compromises both esthetics and function. This clinical presentation will focus on the application of both hard and soft tissue grafting in the esthetic zone implementing state of the art interactive CT diagnosis and treatment planning for optimal esthetic implant reconstruction.
Emphasis will be on indications, graft and harvest site assessment, timing, and use of bioactive modifiers including BMP, PRGF and PDGF. The surgical protocol utilizing allogeneic and autogenous grafts in conjunction with connective tissue grafts, acellular dermis matrix, and related soft tissue procedures to avoid functional and esthetic pitfalls will be featured. Pre and post-grafting prosthetic workup, interactive CT graft assessment and perio prosthetic driven implant planning, stent driven implant placement, and computer milled abutment fabrication including guided surgery will be included.

The Bio-Derm Ridge Plumping Technique for Pontic Site Development
Seibert Class III apicocoronal and buccolingual alveolar ridge defects with associated gingival mucosal atrophy and absence of interdental papillae are common in edentulous areas within the anterior esthetic zone of the maxilla. Normal emergence profiles, critical to achieving
esthetic restorations, require restoration of
normal hard and soft tissue morphology, including
re-establishment of adjacent interdental papillae.
