Great question. Yes the images are physically moving on the hard drive through LR into the specific patient’s folder. The folder panel on the left side of the screen gives you access to your hard drive and LR tracks the new location of the images. If you do this outside of LR by moving images on the hard drive directly LR will show a “?”indicating the image(s) are not where they were originally placed.
Lets say you added some images to a patients folder directly on the hard drive (x-rays or images emailed to you from another office) outside of LR. When you launch LR and click on the patients folder the images will not appear since they weren’t imported through LR. In this case select from the menu bar; Library>Synchronize Folder and LR will add the new images to the catalog!
