Tom; Dr. Han is retired from dentistry now and no longer manages his posts. Some answers for the board based on my own review, sutures 5.0 monofilament and horizontal mattress. This technique would not work well under an existing Pontic and there are better techniques for adjacent root coverage. Best with autologous grafts, less so with ADM. The tissue can be harvested from palate or Tuberosity. Anyone interested Ina hands-on course go to Live course area on DentalXP. Thnx Dr. Salama

I would be interested in how you sutured the connective tissue in place as well as the suture technique to keep the elevated tissue in place.....horizontal mattress sutures?? Where,5-0,6-0?

Dr. Han, Would this technique also work to augment the ridge under a pontic as well as achieving root coverage of the adjacent natural teeth? Would it work with ADM? Thank you.

The ideal grafting material should be biocompatible, biodegradable, easy handling, cost-effective, minimize inflammation and foreign body reaction, not transmit diseases and assure a long-term restoration and preservation of the alveolar ridge. Is there any better scenario to prove the grafting material properties than the worst scenario in esthetic implant dentistry: central-lateral, site by site? Negative Factors for Soft & Hard Tissue Maintenance
Maintaining the bone is the most difficult challenge in implantology (bone grafted or native bone around implants). If a tissue want to live long, it has to follow 2 conditions: The first condition is to organize a full blood supply.. However, it’s not enough.
The solution for the long term stability is to try to organize the stability of the blood supply.. by the respect of several biologic conditions. Almost of these conditions are explained in this lecture.
We introduce here the new concept to avoid the reduction of blood supply by the periosteal incision: the soft brushing technique is the first technique which allows a very large increase of the flap without any incision: the flap closure without tension but without any incision.

A Novel Approach to Root Coverage: The Pinhole Surgical Technique
Free connective tissue graft techniques are currently considered the most predictable surgical method for root coverage. However, morbidity associated with secondary graft sites has generated interest in other methods. The purpose of this study was to investigate the feasability of a novel surgical approach to root coverage: the pinhole surgical technique (PST). The retrospective study examined the results of PST used for 43 consecutive patients on 121 recession sites, of which 85 were Class I or II and 36 were Class III. The documented results indicate that PST holds promise as a minimally invasive, predictable, effective, and time and cost-effective method for obtaining optimal patient-based outcomes.
