Immediate Dentoalveolar Restoration - Labial Contour Grafting at the Time of Immediate Placement
Until recently, tooth replacement at compromised sites in the aesthetic zone, required long-term treatment with possible undesirable complications in the tissue architecture. The Immediate Dentoalveolar Restoration (IDR) is a technique established to broaden indications for immediate loading on individual teeth. In this way, tissue losses with varied extensions are reconstructed in the same surgical session of implant placement and provisional crown installation, reducing the number of interventions and keeping predictability on aesthetic aspects. The IDR, which advocates minimally invasive procedures, will be presented as a viable and reproducible alternative.

"The Great Debate" on Tooth Replacement
Bucco-palatal collapse of the post-extraction ridge is a significant challenge in restorative and implant dentistry. A variety of different approaches as well as ridge preservation techniques using tissue and augmentative materials have been proposed in this session with some of the leading clinical research teams in the field. A conceptual debate format discussing the virtues of "Early Placement" at 6 weeks (Buser & Belser Team) vs. "Immediate Placement with Dual Zone Protocols" at time of extraction (Chu & Tarnow Team) vs. "Partial Extraction Therapy" PET (Gluckman & Salama) also at the time of extraction is led by Professor Homa Zadeh of USC. This interactive session explores the current status and poses questions for the future use and indications of each proposed therapy.

Advanced Bone Grafting Techniques: Part 1 - Autogenous Bone Blocks in the Reconstruction of the Atrophic Mandibular Ridge
This lecture will describe the use of autologous onlay block bone grafting for reconstruction of moderate to severe atrophic posterior mandibular alveolar ridge. This will include a discussion of the various options to this form of grafting, such as Short implants, Lingual positioning, nerve re-positioning, distraction osteogenesis, and GBR. The Mutli-Tier onlay Bone Graft Technique will be highlighted in this presentation. Additionally, a review of a recently submitted research paper of long term follow up of implants placed into autologous bone blocks harvested from the mandibular ramus, retromolar and sympheseal areas will be discussed.
A description of surgical donor harvest sites in the mandible will also be performed as well as post op healing of these sites. Additional time will be spent reviewing causes of failure with bone blocks and implants placed into these sites.

Alternative & New Tendencies for Alveolar Reconstruction in Modern Implant Dentistry

Post-Traumatic Treatment of Maxillary Incisors by Immediate Dentoalveolar Restoration with Long-Term Follow-Up
Replacing both missing maxillary interior teeth is particularly challenging, especially in compromised sockets. The case report describes the management of an 18-year-old female patient, who suffered avulsion of both maxillary central incisors at 7 years of age. The multidisciplinary implant technique, called Immediate Dentoalveolar Restoration (IDR), included extraction of the injured teeth and a single procedure for immediate implant placement and restoration of the compromised sockets after root fracture and peri-apical lesion development were detected during orthodontic treatment. Successful esthetic and functional outcomes and reestablishment of the alveolar process after bone reconstruction were observed during the 3-year follow-up period. The predictable esthetic outcomes and soft and hard tissue stability that can be achieved following IDR are demonstrated.

Recombinant Human Platelet– Derived Growth Factor: A Systematic Review of Clinical Findings in Oral Regenerative Procedures
The use of
recombinant human plateletderived
growth factor–BB (rhPDGF)
has received Food and Drug
Administration approval for the
treatment of periodontal and
orthopedic bone defects and dermal
wound healing. Many studies
have investigated its regenerative
potential in a variety of other oral
clinical indications. The aim of this
systematic review was to assess the
efficacy, safety, and clinical benefit
of recombinant human plateletderived
growth factor (rhPDGF) use
for alveolar bone and/or soft tissue
regeneration. Based on the
clinical evidence, rhPDGF is safe and
provides clinical benefits when used
in combination with bone allografts,
xenograft, or β-TCP for the treatment
of intrabony and furcation periodontal
defects and gingival recession or
when used with allografts or xenograft
for GBR and ARP.
