Rehabilitation of the Extreme Atrophic Jaws

Description:
In this lecture you will learn several alternatives for the full arch rehabilitation of the jaws with different degrees of resorption. Several cases will be presented step-by-step with high-quality pictures for each technique explained. The main goals are to avoid complex regeneration procedures, to avoid zigomatic surgery and to still be able to perform immediate loading in the most compromised scenarios. All-on-Four technique, palatal approach technique, pterygoid implants and the most recent Subperiosteal Implants are some of the key-concepts presented.

I only did a handful of upper subs in the '80-90's. But many lowers .Lowers were successful.Many are over 25-30 yrs in function. Uppers were not.I did both cast surgical Cr-Co and HA-coated Cr-Co as taught by Misch, Golic, Tatum, Hahn, Heller and others. Most subs were done by 2 appointments. Open surgical bone impression and then appt. 2 for surgical emplacement. These subs fit extremely well and usually snapped to place with intimate bone contact and no visible movement. Ti screws were used to insure no movement. I have not done any subs in years. I would consider a lower done from 3D model with todays technology. My concerns with this system are: 1. Is there enough metal to withstand the forces of occlusion? 2. Will the bone-implant interface be integrated or tissue encapsulated? 3. Can the Ti metal frame and screws withstand years of function or will it fracture or loosen from strain and micro movement? 4. Will the framework eventually loosen and eventually become inflamed and infected?
