前歯部インプラント審美における組織の形、色、厚みの重要性 Part2
Description:
抜歯のプロセスは、抜歯窩の骨再生のイベントに大きな問題を生じることがある。それはたんに頰側骨板を破壊するだけでなく、審美性に関係するソフトティッシュを菲薄にしてしまう。
ソケットに即時にインプラント埋入をするのか、または骨移植をするのかという決断は多面的である。最終的な審美結果に影響を与える要因は、骨移植、インプラントにサポートされるプロビジョナルの使用、ソフトティッシュグラフトがその治療に含まれるかどうかということである。このような審美的結果は、とくにソフトティッシュの形状、色調そして厚みを計測することで様々なアプローチの予知性を高める可能性がある。このプレゼンテーションでは、審美インプラント修復に関するいくつかの側面と多施設研究の新しい知見について紹介する。

Socket Shield; What Do We Know, What´s New?
The Socket Shield technique is a “new” procedure with 10 years of documented successful cases worldwide. Lately we have done some new proposals like the way we extract the lingual slice, the relation of the shield level to the bone, the use of miniflaps, the distance between implant and shield just to mention some.
On the other hand, the only minor “problem” with this technique is the “external shield exposure” and here I propose the method of successful treatment.
We will see the convenience of applying SS to molars also on the light of the paper of Dr. Mataraso from 2009. The molar SS is a viable alternative rather than ridge preservation techniques.

Clinical Outcome of Submerged vs. Non-Submerged Implants Placed in Fresh Extraction Sockets
The aim of this study was to compare the clinical outcome of submerged vs. nonsubmerged
tapered implants placed into fresh extraction sockets. The results showed statistically significant differences between the two groups in
the mean value of keratinized tissue (KT) height after surgery that was significantly reduced
for submerged implants when compared with transmucosal implants. Similar outcomes were found for submerged and non-submerged implants
placed in fresh extraction sockets with a horizontal peri-implant defect smaller than 2mm,
except for a reduction of KT in the submerged group. Either with a submerged or a nonsubmerged
procedure, 1mm of mean soft tissue recession is seen after 1 year when compared with the pre-extraction situation.
