In dental esthetics the regeneration of a compromised papilla , remains the most complex aspect of any reconstructive endeavor. The original 5mm Rule was found to exist only for a papilla bordered by 2 teeth and was expanded into a classification of 6 Types dependent on alternative adjacent support.e. implants or pontics. While different surgical techniques can restore most aspects of a ridge deficiency,as part of the soft tissue restorative frame, the regeneration of lost proximal papilla form is surgically limited to those papillae bordered on each side by a pontic The Classification of 6 types of papilla defects is based on the specifics of the restorative support on either side of the individual site,and can therefore be a tooth, implant or pontic, or combinations of the 3. The resultant possible 6 combinations or classes of papilla have different mean "predictable achievable papilla height dimensions" which, range from 3 mm between 2 implants to more than double that between adjacent pontics, where it may exceed 6 mm.

very nice presentation, thanks a lot. just i want to ask about two things: 1- usingvital submerged root. what about liability of pulp death ,infection, affection of adjacent implant... also are there any liability of any pathologic cyst formation from surrounding periodontal ligament. 2- last faces of composite built up... are there any problem of plaque accumulation on this shelf?

I am interested in guided subgingival augmentation (GSA), my question is: can we build papilla more than 3mm between 2 implants if we do GSA right after implant placement then after 3 months we do gum windowing for taking impression and make temporary crown (screw retained) to build papilla. Thank you

Combination Therapies in the Esthetic Zone: Steps for Success
This presentation will cover the critical aspects to consider for deciding the sequence and implementation of combination therapies. Successful esthetic zone treatment must begin with a well thought-out space management plan. This plan involves a three dimensional adjustment of the periodontal attachment, with a possibility of root submergence and partial extraction technique. Tooth preparations without a defined finish line have been termed in several different ways, such as knife edge, feather edge, or shoulderless. From a biological standpoint, preserving a maximum amount of sound tooth structure, as it is done in vertical preparations, might also offer a more conservative alternative to a horizontal margin design in other clinical conditions such as endodontically treated teeth, vital teeth in young individual, teeth affected by caries at the cervical third. Vertical margins on zirconia crowns have been tested in vitro and clinical reports have been published. A re-evaluation of possible advantages and shortcomings of vertical preparation design in contemporary prosthodontics will be presented in light of recently published literature.

Surgical Veneer Grafting - Compensation for Natural Labial Plate Remodeling After Immediate Implant Placement
Contemporary implant therapy aims to provide highly esthetic and predictable treatment outcomes while decreasing treatment duration and complexity. The clinician must therefore be cognizant of circumstances with a predisposition toward esthetic outcomes and treatment plan accordingly. Preservation of the surrounding hard and soft tissues associated with an immediate postextraction socket implant to replace a nonrestorable tooth in the esthetic zone is one of the greatest challenges facing the dental team. A case report of a hopeless maxillary left central incisor in a patient with a thin periodontal phenotype illustrates this new surgical and prosthetic approach. Clinical, radiological, and esthetic parameters were recorded to evaluate primary treatment outcomes.

Influence of Thin Mucosal Tissues on Crestal Bone Stability Around Implants With Platform Switching: A 1-year Pilot Study
The aim of this pilot study was to determine what effect thin mucosal tissues can have on crestal bone stability around implants with platform switching. Within the limitations of this pilot study it can be concluded that implants with platform switching did not preserve crestal bone better in comparison with implants with traditional implant-abutment connection if, at the time of implant placement, thin mucosal tissues were present.
