Henry, With this system I have found that the crestal threads allow for good stability in staged implant surgery and the apical threads provide for stability in immediate implant situations. Adequate insertion torque is something I feel strongly about and was initially concerned about with this implant. With some tweaking of the drilling protocol I have not found this to be the case.

Charles, I did get a tremendous amount of information on tantalum and it application to vitreous carbon while in Parsippany, NJ at Zimmer manufacturing plant but I am not sure of the exact method that the titanium is fused to the tantalum sleeve. I can say that I have found this implant to be as strong as the TSV implant and am only minimally concerned about galvanic reaction between these 2 very inert materials.

Hi Richard, my compliments on very nice presentation. I'm interested in knowing your thoughts conceptually on how the loss of threads, that would otherwise be in the area taken up by the trabecular metal, effects initial primary stability and early loading? thanks

Dear Sam, My experiences with the trabecular metal implant have all been favorable but I have not loaded any in 2 weeks. I leave all provisionals just out of contact in centric occlusion and excursions.

Richard,nice video. Immediate screw retained provisionals are ideal when possible. Do you know how the these trabecular metal implants are manufactured. It seems to me these implants are constucted with titanium and tantalum. I am curious how the two metals interphase (Strengh? galvanic reaction?). The Orthopedic trabecular metal components are only made of tantelum. I would be interested to hear your thoughts. Thank you again. Chuck

Dear Mauricio, I think the risk of peri-implantitis is very low due to the tantalum surface being present along the body of the implant and not at the crest.

Dear Alper, I am very optimistic about the application of this material to a dental implant. My early experience is positive and I feel that the 'ingrowth' versus 'ongrowth' component of these implants changes the discussion from BIC to bone incorporation. Clinically, my early success rates have been exceptional. More time and data is needed to determine how it performs under function.

Dental implant success today is judged not only by osseointegration but also by bone, tissue stability and of course long term esthetic results. Cosmetic predictability can often be difficult to attain, and esthetic implant failures can be multifactorial and patient management issues. Once esthetic implant failures occur, many cannot be fully corrected. Some complications must be addressed by an interdisciplinary dental team. In this summary of case reports, surgical considerations are provided, including cases of facial asymmetry/recession due to facial implant placement or bone loss resulting from technique/treatment failures, as well as papillary deficiencies. Restorative considerations for correcting failures are also discussed.

We are aiming for minimal invasive techniques for the benefit of our patients. Nevertheless, there are many occasions in which we need to repair the anatomy and topography of the atrophic site for better positioning of implants and for their longevity. This step by step surgical technique video presents the treatment sequence for recovering severe vertical and horizontal posterior maxillary atrophy for optimal implant placement.
Vertical augmentation is achieved by using:
First the anatomy: performing sinus augmentation via lateral wall technique.
Second, autologous bone block graft for further recovery of the vertical deficiency decreasing the enlarged inter-maxillary distance. Horizontal augmentation is achieved by using autologous bone block. 
Part II will depict implant placement and soft tissue manipulations.

Ten hydroxyapatite-coated cylindric implants, 13 to 15 mm in length, were placed together with a composite bone graft of demineralized freeze-dried bone allograft and autogenous bone in 10 adults. Surgical technique and anatomic considerations are discussed. Follow-up of 3 years showed successful function and no cervical bone loss in all patients.

Esthetic Rehabilitation of the Periodontally Compromised Dentition - A Novel Interdisciplinary Approach Using Orthodontic Extrusion and Dental Implants
Rehabilitation of esthetics in advanced periodontal cases presents a major challenge in dentistry today. Patients with advanced disease affecting teeth in the esthetic zone, where significant asymmetrical bone loss and tissue recession are present, constitute a challenging treatment-planning dilemma when choosing the optimum therapeutic option for long-term maintenance. This article reviews a novel interdisciplinary approach for vertical augmentation of bone and soft tissues by means of orthodontic extrusion of severely periodontally compromised teeth, facilitating the restoration of esthetics with improved bone and soft tissue volume around immediate implants.
