クレスタルアプローチにおけるシュナイダー膜の伸展率の評価と新しい手術法
Description:
クレスタルアプローチにおいて、挙上されたシュナイダー膜は伸展しているはずである。その伸展率はドーム状に挙上された表面積を底面積で割ったものとして表せることができる。
伸展率が高ければ高い程、シュナイダー膜は伸ばされていることになりシュナイダー膜のパーフォレーションの危険が高くなる。
伸展率を低く抑えるには、底面積を広くとることが重要であるが、圧力によってシュナイダー膜を上顎洞底から剥離するだけでなく、インスツルメントによって確実な剥離も同時に行うことは有利である。さらに小さなホールからアプローチするより長さのあるスリットの方が底面積を広くすることができる。このことより、スリットからのアプローチする「スリットリフト」という手術法を開発した。演者は「スリットリフト」により手術の成功率が大きく向上した。その手技を図および手術ビデオにて解説する。

Dr.Fells Koerniadi,I achieved the primary stability by two point. First, the slit width is about 3mm and the implant is 4mm in diameter. Second,I use a large size cap for the primary stability.
