Smile Design 3D vs. 2D: The Evolution of Precision in Esthetic Digital Dentistry

Description:
In the digital dentistry era, trials never stop to achieve a full digital workflow to fabricate aesthetic restorations based on predictable and accurate facial guided smile design methods but the existing techniques present limitations in the process of transferring the design from 2D to 3D as they are mainly based on 2D Photography and 2D to 3D superimposition and transfers. In this presentation a review of the available techniques and technologies is presented as well as a novel technique that has been used to design a 3D smile frame using 3D face scans, 3D intra oral optical scans, CBCT and multiple software to overcome the limitations of already available 2D smile design systems offering predictable, more accurate and easier to use workflows for success in analysis, planning, design and execution in everyday clinical practice.

Thank you for a very informative presentation. Digital dentistry is certainly the future. However, areas such as implant treatment have benefited hugely from the new technologies. It appears to me, that the digital smile design as presented , relies too much on software and equipment. The best 3 d image is still the patient and a diagnostic wax up can be made quite easily with a lecron and a flame. This gives me an instant trial with a key and i can show the patient how the new smile will look like and i can assess tooth positioning ,tooth length etc and smile line. I also use the wax up as a guide to tooth preparation . The moment a new patient comes to my clinic and we have an informal conversation, I see straight away the kind of parafunction I will be dealing with. This technology seems to take the patient personality and their wishes out of the equation. There is an art to esthetic dentistry , which makes it so enjoyable and professionally very rewarding.Just my thoughts....

The computer guided implant surgery and the x-guide navigation systems introduced a novel minimally invasive concept in the treatment of total edentulous and terminal dentition patients, with new perspectives based on bone graft-less rehabilitation and low morbidity implant surgery. Moreover with diagnostic data obtained from CBCT and extra and intra-oral optical surface scanning, a novel digital pathway based on facially driven virtual diagnostic waxing, prosthetically driven surgical plan, digitally printed surgical template, and prefabricated implant-supported screw-retained interim restorations can be realized in a predictable manner. The challenging interplay with the surrounding gingival framework, often overlooked in the extensive rehabilitation supported by tilted implants, will be highlighted through a step by step clinical protocol to deliver a natural gingival architecture and a direct pink free prosthetic emergence.

Controlled Ridge Splitting (CRS)
To demonstrate a new technique of controlled ridge splitting (CRS) in severely atrophied maxillary cases as an alternative to autogenous block graft. Twenty cases were completed using a controlled ridge splitting (CRS) technique with a total of 65 implants were placed in severely atrophied Maxillae and followed after the implants were loaded. Results: The CRS technique was used in very complex cases, where the alternative method will be autogenous block graft. A total of 65 tapered implants were placed. The implants diameter ranged between 3.25-5mm with a length ranged between 10-13mm. The implants were restored and were followed for 1-3 years. All implants achieved osseointegration and continue to have successful prostheses. Conclusion: The CRS is a predictable treatment option and could be a good alternative to autogenous block grafts is severely atrophic maxillae.
