Description:
Titanium mesh has been used in reconstructive surgery and ridge augmentation for several years. The traditional technique involves using cancellous autogenous bone under the mesh scaffold. More recently growth factors and bone substitutes have been utilized as autograft replacements. There are several advantages to using titanium mesh for space maintenance including strength, ease of use, low cost and biocompatibility. However, the difficulty in removing the mesh has led to a search for other space maintenance scaffolds including tenting screws, dense preshaped collagen and porous high-density polyethylene. This webinar will discuss the use of rhBMP-2 for managing horizontal and vertical bone deficiencies with various scaffolds.

Dr. Misch, thanks for your wonderful and clear presentation. Here is my question, if you use Ti Mesh in large maxillary defect like the case you showed (one side with Ti mesh, one side with particulate graft and collagen membrane) , because we know it's better not to give the patient removable temporary prosthesis because it may cause too much pressure on the grafting site, I wonder what kind of temporary prosthesis will you use for your patient with esthetic concern?

The “classical” subperiosteal implant does not enjoy a good reputation; the causes of failure were myriad, as evidenced in the literature. When considering the future of implants, it appears justified to reconsider the old concepts in light of the availability of contemporary technologies, such as computer-aided design, softwares and programs design, virtual stress– strain testing, and 3D printing of titanium alloy. With the employment of these modern technologies, the concept of a “high-tech” subperiosteal implant has gradually emerged. Fibrin-Guided Bone Regeneration: Guidelines of a Simple, Predictable, & Low Risk Technique
This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.

Autologous bone grafting used with dental implants was originally described by Bränemarketal in 1975, and is now a well-accepted procedure in oral and maxillofacial rehabilitation.Placement of an endosseous implant requires sufficient bone volume for complete bone coverage. Furthermore, the pattern of ridge resorption contributes to an unfavorable maxillomandibular relationship, requires angulations of the implants and/or angled abutments, and affects the proximity of adjacent facial concavities(maxillary sinus, nasal cavity) and vital structures(mandibular nerve).
