Total Rehabilitation of the Edentulous Patient with the All-on-4® treatment concept

Description:
A fixed implant supported prosthesis is the ideal treatment option for the edentulous patient and those facing the imminent loss of their remaining dentition. Through innovation, along with continuous clinical and scientific research, it has become possible to treat virtually every edentulous case with a highly functional and esthetic fixed solution. The oral rehabilitation of these patients has two stages: surgical and prosthetic. The surgical protocol follows the All-on-4® treatment concept - an innovative technique developed in the early 90’s at the MALO CLINIC that allows for immediate function, full-arch, implant supported prosthetics. This widely applicable surgical protocol allows for a simple and immediate rehabilitation with a provisional bridge fixed on 4 implants, with minimum cantilevers, 3-4 hours after the surgery. The final prosthodontic work is of extreme importance. The MALO CLINIC Ceramic Bridge is the epitome of biomedical engineering combined with NobelProcera® CAD\CAM technology to provide patients with an extremely esthetic precision implant supported functional replication of the natural dentition.

Saleheh; Zygomatic and Pterygoid implants require special training and skills. We would suggest that this is NOT a routine procedure but only for very experienced and well trained surgeon and prosthetic teams. regards Dr. Salama

Zero Bone Loss Protocol: Influence of Vertical Soft Tissue Thickness on Crestal Bone Stability
Crestal bone stability is considered to be important for bone preservation, longevity of implants and to prevent peri-implant tissue recession. Implant placement level is important for bone stability. Mucosal tissue thickness was shown to be the factor having impact on bone stability. It was showed that platform switching does not reduce crestal bone loss, if soft tissues at the implant placement are thin. It is suggested that thin tissues might be thickened during implant placement, thus reducing bone resorbtion. The role of soft tissue thickness is well established in aesthetic treatment. It is known that thin soft tissues present an unfavorable situation for implant placement, crestal bone stability and subsequent prosthetic treatment.

Current Concepts for the Preservation of the Post-Extraction Alveolar Ridge
Different techniques have been presented in literature in order to preserve the post-extraction site; including the use of barrier membranes, tri-dimensional matrix and bone fillers. An “open-healing” approach is nowadays commonly used, combining a proper management of soft tissues with biological integration of biomaterials. Once hard tissue has been successfully augmented, an adequate soft tissue thickness becomes mandatory in order to create healthy and stable peri-implant tissues. An innovative volume-stable collagen matrix provides the opportunity to treat soft tissue defects around implants as an alternative to connective tissue graft.

The purpose of this article is to prospectively assess the outcome of immediate rehabilitation of extremely atrophic mandibles by a full-arch fixed bridge anchored to four implants. All patients were followed for a minimum of 1 year (range 20–48 months, mean 30.1 months). No failures were recorded to date. The 1-year implant survival rate and prosthesis success rate were 100%.Marginal bone loss around axial and tilted implants was similar at 12-month evaluation, being, respectively, 0.6 1 0.3 (standard deviation) mm and 0.7 1 0.4 mm. High patient’s level of satisfaction was recorded for function, phonetics, and aesthetics.

Biologic Width Adjacent to Loaded Implant with Machined and Rough Collars in the Dog
Dental implant surface technology has evolved from a relatively smooth machined implant surface for osseointegration to more roughened osteoconductive surfaces. Recent studies suggest that peri-implant soft tissue inflammation with progressive bone loss (ie peri-implantitis) is becoming a prevalent condition. One possibility that could explain sucha a finding is that more bacterial plaque forms on the roughened implant and abutment surfaces, which may result in the peri-implant inflammation if the soft tissues. This study compared 36 tissue-level implants with a machined transmucosal collar to 36 implants with a relatively roughened transmucosal surface in the dog. The results demonstrated that the connective tissue contact was similar between the two implant types but that the junctional epithelium and biologic width dimensions were greater around the implants with the machined collars...
