Transitioning of Teeth to Implants: Risk Assessment for Successful Outcome

Description:
In the transitioning of teeth to implants, a number of critical risk factors have to be assessed. Dozens of decisions have to be made by clinicians based on consideration of factors, including: esthetic requirements, morphology of extraction socket, biotype of peri-implant soft tissues, implant macro design, implant length and diameter, implant surface texture, management of the horizontal gap between socket and implant, provisionalization and loading protocol. Some of these decisions are guided by scientific evidence, while many of the decisions are typically based on empirical guidelines set by individual opinions. This presentation will provide a rationale and outline a predictable protocol for replacement of teeth with implants.

Nice job Homa! Beautiful results as usual. Do you see similar results with immediate placement if you use a screw retained provisional vs. direct abutment and cementing? This takes the potential of cement retention out of the surgical site and then still allows transition to the custom abutment. Barry F.

Current Concepts in Immediate Implants
Immediate implants have several advantages over traditional phased implant placement. Shortened treatment time and a single surgical procedure are among some of these benefits. Although the surgical techniques can be more challenging, immediate implants can be a predictable treatment modality in both the esthetic zone and molar sites. Interpositional Bone Grafts to Treat the Posterior Atrophic Mandible
This presentation analyzes the interpositional bone graft or inlay technique, discussing the reconstructive surgical approach. Horizontal osteotomy with the interposition of bone in the form of a “sandwich” involves raising a coronal osteotomized segment of the mandible, which is still attached to the lingual periosteum, and interpositioning a block bone graft. This technique guarantees a dual vascular supply to the inlay graft from the lingual periosteum and from the residual bone; it also allows optimum use of the native basal bone, which should be less prone to resorption. The advantages and disadvantages of the inlay technique are compared with other commonly used augmentation techniques in the management of posterior mandibular atrophies.
