The Use of Triple Graft for Compromised Alveolar Sockets Associated with Gingival Recession
Description:
The Immediate Dentoalveolar Restoration technique has been used for over 7 years, allowing the reconstruction of tissue losses in the same surgical session of dental extraction, implant placement and provisional crown installation, minimizing the treatment time and morbidity of the procedures.
This presentation shows the triple graft protocol and the result of hard and soft tissues stability when the IDR technique is applied. The triple graft contains cortico-cancellous bone and connective tissue in the same piece and it is indicated for cases of total or partial loss of buccal bone wall associated with gingival recession.
To make the triple graft viable, some changes must be made to the IDR cortico-cancellous graft protocol, particularly in the way that the graft is harvested from the donor area and is stabilized in the receptor site.

kevin potocsky - First of all we must compact the bone particles over the surface of the implant covering partially the bone defect. After that the triple graft will be modeled according to the shape of the defect and sutured in the flap thus obtaining an excellent primary stability.

Zero Bone Loss Protocol: Influence of Vertical Soft Tissue Thickness on Crestal Bone Stability
Crestal bone stability is considered to be important for bone preservation, longevity of implants and to prevent peri-implant tissue recession. Implant placement level is important for bone stability. Mucosal tissue thickness was shown to be the factor having impact on bone stability. It was showed that platform switching does not reduce crestal bone loss, if soft tissues at the implant placement are thin.
It is suggested that thin tissues might be thickened during implant placement, thus reducing bone resorbtion. The role of soft tissue thickness is well established in aesthetic treatment. It is known that thin soft tissues present an unfavorable situation for implant placement, crestal bone stability and subsequent prosthetic treatment.
