The Utilization of Advanced Flap Management & GBR to Large Alveolar Ridge Defects

Description:
Proper flap management is critical and key to obtain tension-free flap closure at large 3D ridge augmentation sites. The objective of this presentation is to highlight the most relevant clinical aspects to reduce tension during wound closure, therefore reducing associated complications, namely flap dehiscences that predispose our patients to post operative infection and reduced bone formation. Several clinical tips will be highlighted via multimedia and video to help the clinician increase their own predictability with these kind of procedures. Clinical cases will be presented to demonstrate the steps discussed in detail.

Modern Flap Management in Bone Augmentation
What does it mean “Modern”? Less traumatic? More predictable? More biology? Less incisions? Let’s see what modern means from a biological and from a surgical standpoint. How can we achieve a full passive closure from the flaps without damaging the blood supply? Can we take care of the oxidative stress? Can PRF help us in these matters? Surgically, the management of anatomical elements such as the mylo-hyoid muscle and the mental foramen will be discussed and a simplification protocol will also be presented. No doubt, “modern” flap management raises a lot of questions!

Current Concepts in Bioactivity and Regeneration
An injury to the tissue instantly stimulates the healing process. To heal properly, we need three elements: scaffolds, cells and growth factors. If one of these three elements are missing there will be no tissue regeneration. Plasma rich in growth factors (Endoret® (PRGF®) technology, enables clinicians to apply autologous growth factors and a fibrin scaffold in a simple, safe and effective method. The Endoret® (PRGF®) stimulates the tissue-building cells to achieve regeneration. An optimal alveolar regeneration is characterized by fast haemostasis, adequate inflammation, stem cell mobilization and differentiation, rapid soft tissue closure of the socket and correct tissue regeneration. The adequate management of the extraction socket will determine the final outcome of implant-supported restorations.

Ridge Expansion, GBR & Meshes: Decision Making in Clinical Practice
A variety of bone augmentation procedures are currently available at use to the clinician. Techniques of variable complexity can essentially yield favorable results. The choice of technique to utilize however, is often based on defect site presentation and operator comfort level. This presentation will focus on the use of ridge expansion, guided bone regeneration (GBR), and titanium mesh. Merits and indications of each technique will be discussed, and clinical cases will be presented that showcase the decision making process involved in treatment planning.

Modern Implant Dentistry: Rules of Engagement in the Esthetic Zone - Part 2 of 2
This presentation will focus on the interdisciplinary relationship of the restorative dentist, periodontist and orthodontist to reconstruct the soft tissue foundation for all of these restorative options in anterior tooth replacement. The diagnosis of deficiencies as well as the varied treatment options will be discussed in detail. This includes periodontal crown lengthening, esthetic periodontal plastic soft tissue grafting procedures as well as prescription adjunctive orthodontic tooth movement to manipulate the soft tissue foundation prior to or subsequent with the restorative options of implants, bridges, or pontic replacement.

The Bio-Derm Ridge Plumping Technique for Pontic Site Development
Seibert Class III apicocoronal and buccolingual alveolar ridge defects with associated gingival mucosal atrophy and absence of interdental papillae are common in edentulous areas within the anterior esthetic zone of the maxilla. Normal emergence profiles, critical to achieving esthetic restorations, require restoration of normal hard and soft tissue morphology, including re-establishment of adjacent interdental papillae.

Human Histologic Analysis of Implant Osseointegration in a Healed Site Grafted with Nondemineralized Autologous Tooth-Derived Graft Material

Human Histologic Analysis of Implant Osseointegration in a Healed Site Grafted with Nondemineralized Autologous Tooth-Derived Graft Material
Human autologous tooth-derived grafts (ATDGs) were recently introduced as a source of bone substitute biomaterial. Using dentin autografts in humans was first reported in 2003. In that first report, demineralized dentin matrix granules were used as a bone substitute biomaterial for sinus augmentation. Since then, a variety of TDGs have been introduced clinically, including block or particulate forms of tooth structures with various levels of mineralization, as shown in Fig 1. In the present article, “TDG” is used as an all-encompassing term to better capture the variability of tooth-derived grafts that can contain the total root structure, dentin only, dentin and cementum, or even enamel.
