Paul; The microscope takes time to learn and become comfortable. I use it typically for incisions and suturing in large soft tissue cases. Thanks for the kind words. Dr. Salama

Dr Salama another excellence in mucogingival surgery. do you always use the operating microscope or loops.
how is the operating microscope maneuverability while moving from one site of the mouth to the other beside instrumentation changing.i find this is quite a challenge extraordinary ability. thank you

Ma. Reina; Thank you. First, you must stay in the connective tissue and do not go down to the bone when removing the graft. Second, we cauterize the area with a Laser or Radiosurgical device. Third, we apply collatape onto the wound and secure the edges with cyanoacrylate (medical glue) and Fourth, we secure a Vacuum formed Essix Matrix to provide pressure and as a comfort guard for the following week. Hope that helps you plan your case. Good luck Dr. Salama

"The Soft Block": Treating Severe Recessions Around Dental Implants
Implants have been one of the major breakthroughs in dentistry.
Tissue collapse around dental implants is our number one enemy as it gives rise to serious problems that go from inflammation, through many times chronic infection, all the way down to severe recession, seriously compromising not only esthetics, but also function and our patients well being.
Strategies have been developed to help clinicians to try to avoid collapse. Among the most important are procedures such as: grafting the gap; GBR with several biomaterials and barriers; flapless approaches; immediate implant placement after tooth removal with provisionalization using CAD-CAM & 3D technologies and ultimately PET strategies also combined or not with immediate load

A Microsurgical Approach in Hard and Soft Tissue Grafting in Post Extraction Sockets
Minimally invasive surgery has demanded us to develop innovative tools and techniques and instruments to reduce postoperative complications and enhance faster and better healing in regeneration and reconstruction in hard and soft tissue grafting , especially in the esthetic zone. This case report shows the use of microsurgical approach and intervention to reconstruct the post extraction socket and the soft tissue around it.

Novel Surgical BioSealing & Original BioProfiling Techniques - Part 1 of 2
Biphosphonates Related Osteonecrosis of the Jaw is one of the tragedies affecting our patients each time more often representing a new challenge in what we like to call human dentistry. This severe and unpredictable outcome can evolve from something as simple as a tooth extraction or simply an implant placement.
In this presentation the authors display one of the first BRONJ cases in the world treated with the surgical biosealing technique using the PRGF ENDORET graft. Understanding the BIOlogy behind the healing process has been as much surprising as useful in trying to alleviate our patients pain and improve their quality of life. This video presents a novel technique to the post-extraction site based upon taking care of the overall tissue support from the bone level and we will discuss treatment planning, options and limitations with this technique/protocol.

Microsurgical Soft Tissue Reconstruction for Teeth & Implants - Part 2 of 2
Esthetics plays a key role in a modern society and the average dental health of the population is improving every day. In such environment, the demands of dental treatments are different and dentists should approach their patients in a different way: they should be prepared to implement different and modern techniques around teeth and implants. Soft tissue grafting techniques provide the dentist with an armamentarium that makes possible to prevent and solve many problems and complications around teeth and implants, making more predictable the treatments results.
This presentation is focused in the importance of soft tissue grafting techniques around teeth and implants and how to do it in a predictable, modern and a minimally invasive way. We present many different techniques illustrated with clinical cases.

Modern Crown Lengthening: Classification and Clinical Application
Crown lengthening procedures are indicated to provide adequate tooth structure in case of subgingival tooth fracture or caries, uneven gingival level, un-esthetic short crowns due to the tooth wear, inadequate axial height, altered passive eruption ,forced eruption of a single or multiple teeth and finally in case of gingival smile.
The treatment modality for esthetic crown lengthening procedure should be based on detail diagnosis in each case because of the multifactorial etiology and because of the type of therapy selected by the clinician, will have a direct implication for the esthetic result.

Diagnosis, Planning, & Treatment of Altered Passive Eruption; From a Periodontists Perspective
This presentation will teach how to diagnose cases of altered passive eruption based on the radiographic evaluation (with peri-apical rx and CBCT) and how to plan and document a surgical procedure aimed to ‘uncover’ the beautiful and natural hidden enamel. In a world where esthetics is becoming more and more a concern a simple periodontal plastic surgical procedure can re-establish esthetic and balance to the patients affected by this condition.

The Aesthetic Smile: Diagnosis and Treatment
Until recently, dentists' and the public's concept of dental aesthetics was necessarily limited to alterations of the teeth themselves. Dentists concerned themselves with changing the position, the shape and the color of the teeth -basically restoring missing units or enhancing those already present. For the most part the dentist was forced to accept the pre-existing relationship between the three components of the smile; the teeth, the gingival scaffold and the lips.

Advantages of the Root Submergence Technique for Pontic Site Development in Esthetic Implant Therapy
This article suggests a strategy to provide a more predictable protocol for esthetic implant treatment for multiple- tooth defects using the root submergence technique (RST). The RST maintains the natural attachment apparatus of the tooth in the pontic site, which in turn allows for complete preservation of the alveolar bone frame and assists in the creation of an esthetic result in adjacent multipletooth- replacement cases.

Recombinant Human Platelet– Derived Growth Factor: A Systematic Review of Clinical Findings in Oral Regenerative Procedures
The use of
recombinant human plateletderived
growth factor–BB (rhPDGF)
has received Food and Drug
Administration approval for the
treatment of periodontal and
orthopedic bone defects and dermal
wound healing. Many studies
have investigated its regenerative
potential in a variety of other oral
clinical indications. The aim of this
systematic review was to assess the
efficacy, safety, and clinical benefit
of recombinant human plateletderived
growth factor (rhPDGF) use
for alveolar bone and/or soft tissue
regeneration. Based on the
clinical evidence, rhPDGF is safe and
provides clinical benefits when used
in combination with bone allografts,
xenograft, or β-TCP for the treatment
of intrabony and furcation periodontal
defects and gingival recession or
when used with allografts or xenograft
for GBR and ARP.
