Combination Approach to Site Development: Autologous Tissue

Description:
The ideal grafting material should be biocompatible, biodegradable, easy handling, cost-effective, minimize inflammation and foreign body reaction, not transmit diseases and assure a long-term restoration and preservation of the alveolar ridge. Is there any better scenario to prove the grafting material properties than the worst scenario in esthetic implant dentistry: central-lateral, site by site? Partial Extraction Therapy (PET) is the most predictable treatment option to maintain the ridge dimensions almost completely. If the Partial Extraction Therapy is contraindicated, dentin graft, tuberosity bone and mIVAN technique utilizing the pedicled connective tissue graft fulfill all above-mentioned criteria for the ideal grafting material. Dentin graft and tuberosity are also the first choice materials to be utilized in combination with PET. In this lecture, the combination approach to the site preservation and development utilizing exclusively autologous tissues is explained, based on the available literature and presenter´s experience and shown on numerous central-lateral cases.

Soft Tissue Grafting Around Compromised Dental Implants in the Esthetic Zone
Dental implant esthetics can be challenging, but these challenges are magnified when a patient has had multiple failed implant and bone grafting surgeries.  This video demonstrates the treatment of one such case.  This is a surgical video demonstrating the VIP-CT (Vascularized Interpositional Connective Tissue) graft.  While multiple treatment options exist, this start-to-finish video highlights the patient's initial presentation, the surgical technique, and the final outcome at 2 years.

A Review of Clinical Applications for Soft Tissue Allografts & Autografts: Solutions for Teeth, Implants, and Dentures
The presentation focuses on the clinical applications for acellular dermal graft (ADG) materials. ADGs have been commonly presented and discussed to provide solutions for treating gingival recession. Compared to autogenous grafts, they have the advantage of minimizing pain, discomfort, and surgery time for patients by avoiding an intraoral donor site such as the palate.

Tissue Management and Surgery in Implant Dentistry
The expanding role of biologics and minimally invasive surgical protocols is highlighted in this dynamic presentation. The comparison of teeth to implant sites as well as autologous vs. allograft grafts will be discussed. Esthetic Risk Assessment will be introduced and it's use as a a prognostic indicator.

The Critical “Pink” Interface in Esthetic Dentistry—A Cross-Disciplinary Approach: Options, Limitations, and Solutions
This program is a perspective into how cross- disciplinary interface planning has become and integral part of clinical esthetic dentistry, in particular implant diagnostics. Today it is essential to combine “white” tooth esthetics with “pink” gingival aspects. Understanding the individual prosthetic, surgical, and biologic limitations is part of innovative treatment-planning protocols developed in an approach to provide, simplify, and expedite minimally invasive limited therapy. Preemptive CBCT and 3D CAD/CAM planning of soft and hard tissue procedures, implant placement, ridge reduction, and restorative design now utilize new innovative protocols for the entire restoration from top to bottom - implant, abutment, and restoration - and the interfaces in between.

Biologic Strategies to Enhance Clinical and Aesthetic Success in Oral Implantology
This webinar will discuss clinical strategies for treatment of the complex implant site involving bone grafting and implant placement. Our understanding of cellular pathways, and our ability to control the wound response, expands our capacity to effect better aesthetic outcomes and compress treatment time. The reduction of the inflammatory phase of tissue regeneration speeds up osseointegration, prevents crestal bone loss, maintains papillary form, increases the density of peri-implant bone and increases tissue biotype. This enhanced tissue response allows us to predictably treat even the most complex surgical cases, compress time to final reconstruction, and to maintain these favorable tissue outcomes over an extended period of time. The use of dental implants with advanced biologic features and the incorporation autologous growth factors will be demonstrated.

Plastic-Esthetic Periodontal and Implant Surgery: A Microsurgical Approach - Part 1
This 2 part presentation is a comprehensive overview of the principles, indications, and clinical techniques of plastic-esthetic periodontal and implant microsurgery, focusing especially on minimal soft tissue trauma and maximally perfect wound closure. Microsurgery provides clinically relevant advantages over conventional macro-surgical concepts for regenerative and plastic-esthetic periodontal surgery, especially in the all-important esthetic zone. The microsurgical principles and procedures presented will described using high resolution multimedia.

Soft Tissue Cone-Beam Computed Tomography: A Novel Method for the Measurement of Gingival Tissue and the Dimensions of the Dentogingival Unit
The purpose of the present study was to develop a soft tissue cone-beam computed tomography (ST-CBCT) to improve soft tissue image quality and allow the determination of the dimensions and relationships of the structures of the dentogingival unit. Two separate CBCT scans were obtained from three patients with different periodontal biotypes. The first was a scan following standard methods; however, for the ST-CBCT the patients wore a plastic lip retractor and retracted their tongues toward the floor of their mouths.

Changes in Soft Tissue Dimensions Following Three Different Techniques of Stage-Two Surgery: A Case Series Report
The aim of this case report is to compare the resilts of the increase in keratinized mucosa using three different techniques of stage-two surgery. Thirty-two patients with one to eight dental implants who received prosthetic rehabilitation of the maxilla were included. Patients were divided into three groups based on preoperative anatomical considerations. Stage-two surgery was performed using wither the apically repositioned flap, the roll flap, or an apically repositioned flap combined with a connective tissue graft.

Laser Assisted Gingival Tissue Procedures

Laser Assisted Gingival Tissue Procedures
Soft tissue lasers are increasing in popularity among clinicians in part due to their potential value in preprosthetic gingival procedures. The ability of soft tissue lasers to control moisture and facilitate hemostasis appears particularly promising for clinicians excising gingival tissues, performing esthetic crown lengthening, and using resective techniques for gingival troughing—and these applications will grow as practitioners become more familiar with such technologies. This presentation highlights…
