1-the use fo bti conseder-expanding can create an augmentation of temperature? 2 the tipe of thread is more aggressive for the bone

I know it has been awhile since this procedure and I am curious as to how successful it was. Also, is there a reason you didn't use prgf this time? Thanks.

Danny O'Keefe - (3/14/2010 10:12 AM)

Wow, This should increase my tenacity at trying to get the best result possible. Great presentation and look forward to the future care of this patient Thanks

Mike Sonick - (2/19/2010 4:03 PM)

This case as you all eluded to was very challenging. Presntly the patient is wearing a provisional and is going to Ira Dickermans lab to ascertain the color. The ridge was expanded utilizing the motorized BTI expansion burs. I like to use these to expand the ridge, but also use osteotome (altho more traumatic) and piezosurgery. However, the ridge must be thicker for this technique. Thanks for the nice comments and I will be sure to post the finals once the case is completed.

Excellent surgery. It is gratifying to hear someone discussing the challenges(I would not label them as failures)that we often encounter in our attempts to restore lost anatomy. I face these challenges, as do all of us, but most presentations leave me with the impression that there are no complications. Thank you, Dr. Sonick.

Ray Kenzik - (1/8/2010 1:37 PM)

Thank you for the complete narration. This case reflects some of the challenges that I find in many esthetic zone cases. Any chance to see final restoration some day?

Maurice Salama - (12/31/2009 8:40 AM)

Must agree with Robert. It is the "little things" that separate success from failure. That's why reviewing surgical steps in detail are so important in improving one's technique. This includes reviewing videos on this site and/or documenting and reviewing your own cases.
All the best
Dr. Salama

Great Video Dr. Sonic. I like the narration and the photos of the entire case history. I like the fact that you show the failures or I should say challenges and your determination to provide your patient with the best that dentistry can offer.

Fibrin-Guided Bone Regeneration: Guidelines of a Simple, Predictable, & Low Risk Technique
This lecture is focused on an effective regenerative approach, but at low knowledge. The goal is to provide a new skill in favor of well-established techniques. The Fibrin Sealant (FS) is an additional clot of human origin. It has adhesive, sealant and inductive action stimulating hard and soft-tissue regeneration. The FS, mixed with bone chips and biomaterials, allows a better handling of the graft that, becoming plastic and moldable, adapts perfectly to bony defects without dispersion of granules. It’s effective also on wound protection achieving a faster healing of soft-tissue. This approach is excellent in the socket preservation and in the peri-implant regeneration on "closed" sites where the implants are inside the defects.

Esthetic Zone Reconstruction: Synergy of Hard and Soft Tissue Grafting
The loss of alveolar ridge contour in the esthetic zone compromises both esthetics and function. This clinical presentation will focus on the application of both hard and soft tissue grafting in the esthetic zone implementing state of the art interactive CT diagnosis and treatment planning for optimal esthetic implant reconstruction. Emphasis will be on indications, graft and harvest site assessment, timing, and use of bioactive modifiers including BMP, PRGF and PDGF. The surgical protocol utilizing allogeneic and autogenous grafts in conjunction with connective tissue grafts, acellular dermis matrix, and related soft tissue procedures to avoid functional and esthetic pitfalls will be featured. Pre and post-grafting prosthetic workup, interactive CT graft assessment and perio prosthetic driven implant planning, stent driven implant placement, and computer milled abutment fabrication including guided surgery will be included.
