In this fourth part of the series, Drs. David Garber and Claudio Pinho demonstrate an alternative approach with Direct Composite Bonding utilizing a guided silicone matrix to compensate for malpositioned anterior teeth, caries, and limited spacing. The step by step technique using a laboratory waxed model and chairside fabricated silicone index, to develop the intra-oral composite restorative tooth forms, is sequentially shown. A simplified method of finishing and polishing is shown as part of this rapid, ultra-conservative, visual tooth re-alignment and replacement.

Dr.Garber, great work. The incisal edge on the centrals were already tilted mesio-lingually and by ading the veneers labially they became broader FL. Did you thin the edges from the mesio.lingual to keep the edges as thin as normal? Thanks

it is a potential problem ,but in watching the video Dr Pinho has taught us to work on one tooth at a time and keep it isolated - - then basically complete initial contouring before moving to next tooth
If it happens inadvertently a Cerisaw works at slicing thru that contact area

Nagarajan,- - as you notice the upper right lateral had already had endo - - the other teeth were fine as it was predominantly an additive procedure with Composite, with almost no tooth reduction. .

Fatigue Resistance and Crack Propensity of Novel "Super-Closed" Sandwich Composite Resin Restorations in Large MOD Defects
To asses the influence of conventional glass ionomer cement (GIC) vs resin-modified GIC (RMGIC) as a base material for novel, super-closed sandwich restorations (SCSR) and its effect on shrinkage-induced crack propensity and in vitro accelerated fatigue resistance. A standard MOD slot-type tooth preparation was applied to 30 extracted maxillary molars. A modified sandwich restoration was used, in which the enamel/dentin bonding agent was applied first, followed by a Ketac Molar or Fuji II LC base, leaving 2mm for composite resin material. Shrinkage induced enamel cracks were tracked with photography and transillumination. Samples were loaded until fracture or to a maximum of 185,000 cycles under isometric chewing, starting with a load of 200 N and gradually up to a maximum of 30,000 each...
