Lateral Window Sinus Bone Augmentation with a unique Cartridge Delivery System

Description:
The lateral-window approach for maxillary sinus augmentation was introduced to allow for the placement of implants in maxillary posterior edentulous sites with significant pneumatization of the sinus cavity. Implants placed simultaneously with the lateral window sinus augmentation technique enjoy high success rates that are reported to be similar to implants placed in pristine bone in the maxilla. Traditionally, the main prerequisite for simultaneous implant placement with direct sinus lift has been native vertical bone height greater than 4mm or at least 5mm as measured pre-operatively. Recent clinical evidence suggest that the placement of implants simultaneously with direct sinus lift in ridges with minimum remaining bone may be a feasible treatment modality as long as adequate primary stability can be ensured. In this presentation Dr. Kher will perform the lateral window creation using a Piezosurgical device for creating a bony window and placing CPS bone graft (NovaBone Dental Putty) using the unique cartridge delivery system that significantly simplifies delivery of graft into the Sinus.

Thank you Dr Salama. @Milind, this is Novabone putty. It is calcium phospholoicate. In this surgery I have used it as the sole grafting material. But you could mix it with a xenogratft. I haven't done that so far. I have mixed it with autogeneous bone on a few occasions, but mostly used it as the only graft in the sinus.

Past, Present & Future in Sinus Augmentation: From Lateral Approach to Osseodensification
The lateral window osteotomy (LWO) has been the most frequently utilized and published approach to sinus augmentation, presumably because there are no bone height restrictions and it provides the surgeon with more visual control; it is however, considered to be invasive, time consuming ,expensive and associated with patient morbidity. As a less invasive alternative to the LWO, transcrestal osteotome, balloon and hydrodynamic techniques can obtain a localized elevation of the sinus floor through a 3 - 10 mm diameter crestal osteotomy, minimizing the degree of flap elevation and eliminating the need for preparation of a larger bony window in the lateral aspect of the alveolus. The presentation will show the evolution is sinus augmentation approaches and will focus on current step by step Osseodensification sub crestal sinus elevation with long term follow ups of both clinical and CBCT radiographs.

Maxillary sinus membrane perforation is the most common complication that occurs with sinus elevation augmentation surgery. A technique using a slow resorbing type I collagen membrane for repair of large and complete sinus membrane perforations is described. Sinus Floor Elevation Via the Maxillary Premolar Extraction Socket With Immediate Implant Placement: A Case Series
When immediate implant placement is considered for teeth with close proximity to the sinus floor, apical extension of the osteotomy is significantly limited, and often a staged approach is used. Implant placement into fresh extraction sockets and sinus floor manipulation using bone-added osteotome sinus floor elevation with implant placement are techniques most often used independently or sequentially. In this care report, immediate implant placement with simultaneous osteotome sinus floor elevation is an advantageous combination of two successfully used techniques. This combined approach can significantly reduce the treatment time for implant therapy in teeth with close sinus proximity and provide the operator with the ability to place implants of desired length.
