Description:
Minimally invasive surgery has demanded us to develop innovative tools and techniques and instruments to reduce postoperative complications and enhance faster and better healing in regeneration and reconstruction in hard and soft tissue grafting , especially in the esthetic zone. This case report shows the use of microsurgical approach and intervention to reconstruct the post extraction socket and the soft tissue around it.

Thanks for sharing..like other colleagues have mentioned would love to see the PO.How would the results compare to other non-invasive techniques for socket grafting? Does subjecting the patient to the trauma and morbidity of two donor sites in addition to that at the extraction site provide any added benefit? Thanks

Edison Thanks for the comments . You can use PRF to seal the socket and get great results. The palatal free gingival graft to seal the socket gives more height of the crestal gums and creates a suitable environment for implant placement. Yes you can use only allograft bone mixed with the acellular fibrin glue and you will get similar results, the only difference is that cortical autogenic bone contains BMP. THANKS

Dr. Rabie, could you also eliminate the palatal harvesting by using L-PRF membrane solely to provide the barrier for primary closure? How far can you "push" the L-PRF membrane to accomplish the primary closure?

Nicely detailed presentation of your surgical procedure. I,too, would like to see the definitive healed case with radiographs and clinical pictures. Would one expect to see the same results with the L-PRF mixed with allograft bone particles and thereby eliminating the second surgical site?

Today aesthetics has become the main reason for dental consultation. The appearance and shape of soft tissues are an essential part of achieving smile balance and harmony. In recent years, the use of lasers has evolved significantly in periodontal plastic surgery, due to its applications, such as; the ability to offer treatments without sutures, minor tissue damage, minor bleeding and therefore promote faster tissue recovery. This time we used GEMINI Ultradent double wavelength diode laser, from 810nm to 980nm, super pulsed with a power of up to 20 watts; improving coagulation performance and providing a faster and more precise cut than other diode lasers.

Tooth preparations without a defined finish line have been termed in several different ways, such as knife edge, feather edge, or shoulderless. From a biological standpoint, preserving a maximum amount of sound tooth structure, as it is done in vertical preparations, might also offer a more conservative alternative to a horizontal margin design in other clinical conditions such as endodontically treated teeth, vital teeth in young individual, teeth affected by caries at the cervical third. Vertical margins on zirconia crowns have been tested in vitro and clinical reports have been published. The objective of this article is to demonstrate the benefits of an acellular dermal matrix called PerioDerm™ Acellular Dermis in correcting recession and adding connective tissue during periodontal-restorative comprehensive treatment. 1,2 It has long been established that connective tissue is an integral factor in the protection of the underlying periodontal foundation. Connective tissue tends to be tenacious in makeup and have far less vascularization than mucosa. It prevents bacterial infiltration to the underlying supportive periodontal tissues, and in restorative applications, it adds stability to gingival areas during impression taking and cementation of restorations. Though the quality of tissue is critical, it is also important that the tissue is bound down to either tooth surface or bone to serve as a supportive mechanism for protection of the periodontium.

Recombinant Human Platelet– Derived Growth Factor: A Systematic Review of Clinical Findings in Oral Regenerative Procedures
The use of recombinant human plateletderived growth factor–BB (rhPDGF) has received Food and Drug Administration approval for the treatment of periodontal and orthopedic bone defects and dermal wound healing. Many studies have investigated its regenerative potential in a variety of other oral clinical indications. The aim of this systematic review was to assess the efficacy, safety, and clinical benefit of recombinant human plateletderived growth factor (rhPDGF) use for alveolar bone and/or soft tissue regeneration. Based on the clinical evidence, rhPDGF is safe and provides clinical benefits when used in combination with bone allografts, xenograft, or β-TCP for the treatment of intrabony and furcation periodontal defects and gingival recession or when used with allografts or xenograft for GBR and ARP.
