Description:
Minimally invasive surgery has demanded us to develop innovative tools and techniques and instruments to reduce postoperative complications and enhance faster and better healing in regeneration and reconstruction in hard and soft tissue grafting , especially in the esthetic zone. This case report shows the use of microsurgical approach and intervention to reconstruct the post extraction socket and the soft tissue around it.

Thanks for sharing..like other colleagues have mentioned would love to see the PO.How would the results compare to other non-invasive techniques for socket grafting? Does subjecting the patient to the trauma and morbidity of two donor sites in addition to that at the extraction site provide any added benefit? Thanks

Edison Thanks for the comments . You can use PRF to seal the socket and get great results. The palatal free gingival graft to seal the socket gives more height of the crestal gums and creates a suitable environment for implant placement. Yes you can use only allograft bone mixed with the acellular fibrin glue and you will get similar results, the only difference is that cortical autogenic bone contains BMP. THANKS

Dr. Rabie, could you also eliminate the palatal harvesting by using L-PRF membrane solely to provide the barrier for primary closure? How far can you "push" the L-PRF membrane to accomplish the primary closure?

Nicely detailed presentation of your surgical procedure. I,too, would like to see the definitive healed case with radiographs and clinical pictures. Would one expect to see the same results with the L-PRF mixed with allograft bone particles and thereby eliminating the second surgical site?

The Science Behind the Papillae - Part 1 of 2
In 1992, Tarnow’s study on the effect of the distance from the contact point to the crestal bone on the presence of interproximal dental papilla gave us a new and ground breaking insight into the management of pink aesthetics. Later Team Atlanta furthered that study, analyzing the different measures between teeth, implants and pontics, giving us an even deeper understanding of how bone height influences in papilla growth. In my understanding, this study was one of the most important in the history of dentistry due to its true crossover between dental surgery and the prosthodontic field, combining aesthetics, oral rehabilitation and oral surgery in treatment planning. Never has interdisciplinary dentistry been more valued. This lecture will go over past and present techniques that can be performed to achieve an optimal aesthetic outcome in the managing of soft tissue.

The "Modified Tunnel" Technique and the development of the Lateral Tunnel Approach
The "Modified Tunnel" technique is a method of esthetic connective tissue grafting that protects the interdental papillae. First published in 1999, this lateral tunnel approach uses vertical incisions on both sides of the treatment area. These vertical incisions facilitate placement and positioning of the graft over the root surfaces without the need to disturb the interdental papillae. This technique is very useful in esthetic areas and those that involve several teeth. An acellular dermal connective tissue allograft is an excellent choice of graft material to use with this technique.

In addition to surgical intervention is the creation of the specific emergence profile that is essential in the aesthetic zone. The emergence profile composed of 2 parts, the abutment and the subgingival part of the crown. The shape of abutment can be individually shaped so that it gives natural appearance and varies individually (depending on the depth, angulation and diameter of the implant). Failure of a natural tooth may not permit placement of an implant at the time of extraction due to insufficiency in available bone to house the implant. Reconstruction of the extraction socket frequently involves both hard and soft tissue augmentation to provide a site that can house the implant and ridge contours that mimic the adjacent natural anatomy. This situation becomes more problematic in the maxillary anterior due to the anatomy and the lower density of the bone of the premaxilla. The solution is the interpositional vascularized augmentation neogenesis (IVAN), which consists of hard tissue grafts, various barrier membranes, and closure with the pediculated connective tissue graft (PCTG). The modified IVAN (mIVAN) technique achieves the necessary goals and may be used in both delayed and immediate placement situations.

Esthetic Correction of Gingival Recession Using a Modified Tunnel Technique and an Acellular Dermal Connective Tissue Allograft
Esthetic correction of gingival recession is an important goal of periodontal therapy. This article describes a surgical technique that combines a modified tunnel technique and an acellular dermal connective tissue allograft. With the aid of vertical incisions, a tunnel is created under the buccal mucosa of the affected tooth. These incisions enable easy access for graft placement and create mobility for gingival coronal positioning. The use of an accelular dermal connective tissue allograft eliminates the need for a surgical palatal donor site. This minimizes post-surgical complications.
