In today’s age of implant reconstructive dentistry, the pendulum continues to swing towards extractions, alveolectomy and “All on X”. As practicing periodontist and prosthodontist, we have built a successful career around saving teeth strategically and sometimes for a life time with teeth that have a poor to hopeless prognoses. Each patient should be considered for therapy based on risk assessment and concomitant systemic issues, functional issues, and bottom line an educated decision that they can make after we have provided all of the potential solutions. Dr. Romero and Scheyer have worked together for more than 20 years rehabilitating patients with teeth and implants. We understand the true value of osseointegration, allowing for implant reconstructive dentistry. However, utilizing our specialty training achieved more than 20 years ago, we cherish the opportunity to save compromised teeth, to support long-term provisionals and/or final restorations.
With the dental digital revolution underway, we further embrace comprehensive assessment through airway focused facially generated treatment planning using a digital workflow. We feel that our sequential extraction protocol needs to be shared around the XP world nation so that more teeth are saved to support provisionals and avoid the risk of immediate load and bone reduction, as well as, consider saving teeth for longer periods of time if not for the patient’s lifetime in health and function. The Esthetic Reconstructive TRIAD: "GBR, GTR and Tissue Molding"
Implant dentistry in the esthetic zone is highly challenging. In recent years
there are several studies, well defined approaches available on how to achieve
pleasing outcomes. We must understand the biology of extraction sites and different
treatment options for preserving or even reconstructing bony structures for
predictable implant placement. This clinical and evidence based presentation will
emphasize the importance of proper 3-dimensional implant placement, show different
bone preservation techniques i.e P.E.T. (Partial Extraction Therapy) and how soft
tissue grafting can effect the final end result.
However we must admit that not only surgery but prosthetic skills have a major
influence on our work. Implant placement must be determined with the prosthetic
plan in mind! On the other hand abutment design, platform shifting concept as tissue
molding “tools” and screw retained solutions will be high-lighted with lots of case
presentations.

Site Preparation & Osseodensification in Bone Management
Today's implant arena involves not only the understanding of procedures but also the science behind wound healing.
Often new technologies and instrumentation evolves that have a significant impact on what we are able to do clinically.
