Russ, Henry, PTFE can be temporarily placed into jump gap to fabricate temp directly.

Russ, the best way is to use just an absolute minimum of acrylic/composite as needed to lute the crown former/shell to the temporary abutment. You then take it out of the mouth and fill-in the voids and polish. Hope that helps.

Great lecture with great information. My concern with these types of immediate placement immediate temp cases is using the acrylic to make a temporary while the gap between the implant and buccal wall of bone is present. Has anyone had any problems with the acrylic going apically down the implant and locking on before the gap can be filled? Any suggestions to avoid this from happening?

Can't wait to see the next videos. Fantastic presentation. There was a DentalXP video (wish I could remember which one) where the doctor specifically discussed how important it was to get a surgical guide on the implant placement because it was so hard to accurately place the implant drills where you were trying to get the implant to the lingual when there was basically empty space to the buccal. Without a surgical guide how do you get the implant placed so accurately since you are basically "free handing" the placement?

Pop, recent literature tells us that approximately 90% of buccal plates are 1 mm or less. Therefore, the techniques that were described work precisely because they help manage these very thin buccal plates that even if they do resorb/remodel, they last long enough to protect the graft and allow it to vascularize, mineralize & integrate with the implant while minimizing dimensional changes.

“My First Implant”: Conservative Management of a Failing Posterior Maxillary Tooth
In this 2 Part video case report, “conservative” management of a failing premolar in the posterior maxilla is managed by a Dentist beginner placing their first dental implant at a course at the DentalXP Center in Atlanta. Atraumatic Management of the Extraction Site utilizing Ultrasonic Surgery, Extractor elevators and followed by Socket Grafting with PRGF and mineralized bone allograft is featured in Part 1. Flap Reflection, Drilling sequence, 3D placement of the implant, and simple suturing is highlighted in Part 2. The follow up and evaluation of the healing of the socket graft displayed significant bone volume for implant placement in this conservative staged approach for the “beginner” in Implant Dentistry.

Improving Implant Framework
Passive fit of implant frameworks is related in part to the accuracy of the mastercast. A method of fabricating a verification stent to check the accuracy of the master cast prior to waxing and casting the implant framework is discussed. The methods utilize routinely available materials and do not require any special instrumentation or equipment. The stent can be fabricated either by the laboratory or the dentist.
