Modern Day Esthetic Challenges with Implants in the Esthetic Zone

Description:
Current trends in implant therapy advocate clinical techniques thereby allowing treatment efficiency consisting of condensed clinical procedures, fewer patient appointments, and shortened overall treatment time. Even though there is overwhelming literature to support positive outcomes in regards to implant survival through immediate implant therapy and tooth replacement strategies, esthetics questions still loom, specifically how to handle multiple tooth sites in the esthetic zone. This presentation will address treatment planning strategies with multiple tooth sites incorporating teeth, immediate implants with tooth replacement, and pontics in the esthetic zone. Innovations in implant macro design, to achieve more consistent screw-retained definitive restorations, and devices / strategies in provisional restoration fabrication to save time will be presented.

Dear Stephen Chu: How I enjoyed your lecture, as always with very clear ideas, about the nourishment of perimplant tissues. According to the Borg Group article, they talked of create more space in the area closest to the implant platform. Now, in the article by Pablo Galindo and Alberto Monje, where they said that the distance between the edge of the crown and the implant platform should be at least two millimeters. On the other hand, Tomas Linkevicius and his article on mucosa thickness l, greater than 3 mm to preserve or crest. Well, in my humble opinion and clinical experience, in edentulous areas, where the biological space, the gingiva contour and the papillae have been lost, everything depends on the prosthetic design, the wax up will indicate the zenith and to what depth we will insert the implant, the vertical mucosal tissue and the phenotype. I want to say that it is a three-dimensional, biological and mechanical approach so that all the precepts are fulfilled, that you, in a very didactic way, exposed in your webinar and all the articles that you have made reference. I lacked to say about subcritical design, very well explained by Alberto Monje et al and Ernesto Lee et al. where the convexity or concavity depends on the buccal - palatal position of the implant and the volume of the oral mucosal roof. In conclusion, many concepts that we must take into consideration. Thank you for everything you've done to make us better implantologists. Best regards Alberto Miselli

Soft tissue recession around teeth and implants is an epidemic problem in the field of dentistry. A predictable and long-term soft tissue augmentation outcome is primarily dependent on proper diagnosis, prognosis and treatment planning. Harvesting limitations of autogenous tissue from the palate represent a true challenge when treating patients with multiple recessions or gingival deformities around teeth and dental implants. En la actualidad, los objetivos de la odontología han ido evolucionado a partir de las necesidades y las demandas de los procesos quirúrgicos, así como por los defectos o lesiones del periodonto, sobretodo cuando intervienen factores estéticos, donde se requieren restauraciones prostodónticas con aspectos naturales, que busquen un equilibrio armónico entre la arquitectura gingival y el rostro en sí del paciente, aún más en los casos donde se presenta sonrisa gingival. Es por esto que se han estudiado varias técnicas mucogingivales para recubrimientos de recesiones, cambios de color por biótopo muy fino, aumento de volumen por pérdida del contorno gingival y colapso de los tejidos duros y blandos; siendo la más estudiada el injerto de tejido conectivo, como mecanismo de compensación y estabilidad para contribuir a una sonrisa más armónica y bella a la hora de la rehabilitación con implantes en el área anterior.

Our objective in these 2 sequential lectures was to provide straightforward decision trees related to several clinical scenarios observed in routine clinical practice. Influence of Thin Mucosal Tissues on Crestal Bone Stability Around Implants With Platform Switching: A 1-year Pilot Study
The aim of this pilot study was to determine what effect thin mucosal tissues can have on crestal bone stability around implants with platform switching. Within the limitations of this pilot study it can be concluded that implants with platform switching did not preserve crestal bone better in comparison with implants with traditional implant-abutment connection if, at the time of implant placement, thin mucosal tissues were present.
