What are the criteria in determining if I should use PRF or PRGF? Thank you.

I am looking to buy a piezo unit. DoWell co. which you like has one for 6200 or something but Salvin and most other companies have them for 10-15000. I am a average surgeon who does a slight to moderate amount of this work, which do you recommend I get?

Dear Dr. Darko Veljanovski; The barrier membrane concept is not changing but has never been "clearly" described as to the timing of how long it is required. As to "barrier by bulk" that relies on thickness of material to slow down the migration of soft tissue to the regen site....also never proven. In my practice, a large critical size defect require more time with a barrier membrane and a small size defect may require less time and only PRGF or PRF fibrin membranes with bulk underneath. Thanks for your question and I hope this answer helps. regards Dr. Salama

Great presentation by experts in this field. Just not if it was new, it seems speakers refer to 2010-2011?

Gentlemen; Sorry to hear about the issues watching the webinar yesterday. There was a weather issue in Atlanta that may have caused problems with the Global Server or it could be a slow local internet connection or both. We will check on that today. But this morning, I played the entire presentation and it played very well. We apologize about this inconvenience and value your comments. Hope you will attempt to watch again. sincerely Dr. Salama

I paid to see he pikos salama webinar and there was no sound!!!!!!

Dr. Darko Veljanovski - (2/25/2015 2:47 PM)
Brilliant webinar. Great minds.
Dr. Pikos uses bone substitute as "veneer graft" to increase vestibular thickness in his second case. no membrane. I.think he said " biooss", as a slow resorbing material, is that correct? because bio oss is only spongious. Also, he advocates use of Mineross & ctg without any membrane and has evident results using that protocol. In addition , Dr. Anitua says only prf membrane ( lasting 3-4 weeks) is enough as barrier membrane, which is supposed to last much more than that.
My question is: is the barrier membrane concept changing.? Or is this the Misch's "barrier by bullk"?What is the scientific principle behind graft with no membrane concept?
Thank you.

Adosh; there have been some studies that have used PRF Fibrin alone with CPF for root coverage with some success. I prefer CTG as it allows for increased thickness, greater quantity of KT and more predictable root coverage than PRF alone. thanks Dr. Salama

Pre-Implant Ridge Bioengineering: The Bone Block Allograft Technique - Part 2 of 2
Narrow ridge deficiencies can limit dental implant placement. As well, the pain and various complications associated with chin, ramus and hip bone block grafts for ridge augmentation purposes discourage many from pursuing this method of treatment. The 'bone block allograft technique' helps overcome narrow ridge limitations for dental implant placement as well as minimizing surgical post-op pain, risks and complications. This is an extremely versatile procedure which can be performed in the anterior and posterior sextants of both the maxillary and mandibular arches. CadCam Customized 3D-Printed Subperiosteal Implant
When considering the future of implants, it appears justified to reconsider the old concepts in light of the availability of contemporary technologies, such as computer-aided design, softwares and programs design, virtual stress– strain testing, and 3D printing of titanium alloy. With the employment of these modern technologies, the concept of a “high-tech” subperiosteal implant has gradually emerged. Biological design of subperiosteal implant is mandatory for the success of the treatment, guidelines on architectural changes, topological optimization, and biofunctionalization will be explained. This new era of a “high-tech” customized 3D printed subperiosteal implant leads us to consider this technique s a valuable treatment option for atrophic jaws, avoiding more invasive procedures and giving the possibility of immediate loading, improving the patient’s quality of life.

Alveolar Ridge Regenerative Strategies: Autogenous Bone vs BMP-2
This clinical based presentation will compare the use of autogenous bone vs BMP-2 for alveolar ridge reconstruction. The science, indications, advantages and disadvantages of each approach will be featured. Single tooth to full arch reconstruction cases will also be shown along with understanding the application of non-resorbable vs resorbable mesh barriers for alveolar ridge reconstruction.
