I am looking to buy a piezo unit. DoWell co. which you like has one for 6200 or something but Salvin and most other companies have them for 10-15000. I am a average surgeon who does a slight to moderate amount of this work, which do you recommend I get?

Dear Dr. Darko Veljanovski; The barrier membrane concept is not changing but has never been "clearly" described as to the timing of how long it is required. As to "barrier by bulk" that relies on thickness of material to slow down the migration of soft tissue to the regen site....also never proven. In my practice, a large critical size defect require more time with a barrier membrane and a small size defect may require less time and only PRGF or PRF fibrin membranes with bulk underneath. Brilliant webinar. Great minds. Dr. Pikos uses bone substitute as "veneer graft" to increase vestibular thickness in his second case. no membrane. I.think he said " biooss", as a slow resorbing material, is that correct? because bio oss is only spongious. Also, he advocates use of Mineross & ctg without any membrane and has evident results using that protocol. In addition , Dr. Anitua says only prf membrane ( lasting 3-4 weeks) is enough as barrier membrane, which is supposed to last much more than that. My question is: is the barrier membrane concept changing.? Or is this the Misch's "barrier by bullk"?What is the scientific principle behind graft with no membrane concept? Thank you.

Adosh; there have been some studies that have used PRF Fibrin alone with CPF for root coverage with some success. I prefer CTG as it allows for increased thickness, greater quantity of KT and more predictable root coverage than PRF alone. thanks Dr. Salama
