The Use of Triple Graft for Compromised Alveolar Sockets Associated with Gingival Recession

Description:
The Immediate Dentoalveolar Restoration technique has been used for over 7 years, allowing the reconstruction of tissue losses in the same surgical session of dental extraction, implant placement and provisional crown installation, minimizing the treatment time and morbidity of the procedures.  This presentation shows the triple graft protocol and the result of hard and soft tissues stability when the IDR technique is applied. The triple graft contains cortico-cancellous bone and connective tissue in the same piece and it is indicated for cases of total or partial loss of buccal bone wall associated with gingival recession. To make the triple graft viable, some changes must be made to the IDR cortico-cancellous graft protocol, particularly in the way that the graft is harvested from the donor area and is stabilized in the receptor site.

kevin potocsky - First of all we must compact the bone particles over the surface of the implant covering partially the bone defect. After that the triple graft will be modeled according to the shape of the defect and sutured in the flap thus obtaining an excellent primary stability.

Buccal Sliding Palatal Pedicle Flap Technique for Wound Closure After Ridge Augmentation
One standard approach for wound closure after ridge augmentation is coronal flap advancement. Coronal flap advancement results in displacement of the mucogingival junction and reduction of the vestibulum. In the maxilla, a buccal sliding palatal flap can be applied for primary wound closure after ridge augmentation. The dissected part of the palatal connective tissue is left exposed thus eliminating or reducing the amount of the coronal flap advancement respectively and increasing amount of keratinized gingiva. In combination with guided soft tissue augmentation, this flap design enables a three-dimensional peri-implant soft tissue augmentation.

Almost of these conditions are explained in this lecture. We introduce here the new concept to avoid the reduction of blood supply by the periosteal incision: the soft brushing technique is the first technique which allows a very large increase of the flap without any incision: the flap closure without tension but without any incision.
